for Family Physicians

As primary care evolves in our Canadian health system, there is a special growing need to support family physicians as they experience change in their day-to-day practices as well as in their community leadership roles.

This website: Primary Care Toolkit for Family Physicians, is provided by The College of Family Physicians of Canada (CFPC) to help translate, share and disseminate knowledge and management information.

The CFPC will continue to maintain this toolkit as a resource to family physician leaders and their associates who are experiencing the numerous changes in primary care. We welcome feedback and complementary resources from those using the site.

What's New

The CFPC is pleased to release: Discussion Paper on Patient-Centred Primary Care in Canada: Bring it on Home under Introduction, Canadian Medical Home. The medical home acts as the central hub for the provision and coordination of the medical care services needed by each of its patients.

In order to understand the value of nurses in family practice, the Canadian Nurses Association (CNA) has released its Primary Care Toolkit. This toolkit highlights the roles of family practice nurses and nurse practitioners in primary care. The breadth of responsibilities they can be given, and the types of resources (including business case options) which will support the registered nurse’s role in family practice/primary care settings. The Primary Care Toolkit also provides an additional guide for supporting collaborative teams that include family doctors and nurses working together.
